Robert Smith played 6 games with the Jazz in the 1979-80 season. He was acquired in a trade with the Nuggets. In exchange for Smith, the Jazz gave up two draft picks, only one of which (Ken Green) ever played in the NBA. Green finally saw regular-season NBA action in 1986 with the Knicks. He made 5 FTs (out of 9 attempts) for a 55.6 FT%.

In Robert Smith's brief time with the Jazz, he made 5 FGs (out of 15 FGAs) and was 5-5 on FTs. He averaged 5 PPG for the season, counting his time with the Jazz and Nets that season.
 
Jeff Judkins was drafted by the Celtics. The Celtics got the pick they used to draft Judkins from the Jazz in a February 1978 trade that sent Fred Saunders to the Jazz. Saunders concluded his NBA career by playing 30 games with the Jazz in 1977-78, averaging 5 ppg.

Judkins came to the Jazz as a free agent in 1980. There's not much 5 significance to look at in Judkins' one season with the Jazz. In October 1981, the Jazz traded Judkins to the Pistons for a draft pick that wound up being #55 overall in the 1982 NBA Draft. The player selected with that pick, Jerry Eaves, played two seasons with the Jazz and then signed with the Hawks, where he wore #5.
 
Freeman Williams was born 5/15/1986. He came to the Jazz in the infamous trade that sent Dominique Wilkins to the Hawks. Wilkins eventually played on 5 NBA teams. John Drew also came to the Jazz in that trade. Drew averaged either 5.0 or (4.9) FTs per game his last two seasons with the Jazz.

Williams played in 18 games for the Jazz in the 1982-83 season, averaging 5(.1) ppg.

Prior to coming to the Jazz, on August 5, 1976, the Jazz traded the draft pick eventually used to select Williams to the Lakers as part of the infamous compensation package the Jazz and Lakers agreed to after the Jazz signed free agent Gail Goodrich away from the Lakers. So basically Freeman was involved in the 2 worst deals in franchise history. Even though this is the 5 day thread and not the 2 day thread, I find this worth noting.

Billy Paultz played for 5 ABA or NBA teams, including the Jazz. He finished his career with the Jazz in the 1984-85 season. He averaged 5 PFs per 36 minutes and his PER was 5.0 in his lone season with the Jazz. Paultz also played in 5 playoff games with the Jazz, grabbing 5 defensive rebounds those playoffs. Paultz signe with the Jazz as a free agent. To compensate the Hawks, the Jazz gave them a draft pick used to select Sedric Toney, who would play parts of 5 seasons in the NBA.

Paultz is perhaps best remembered by Jazz fans for being hit by Hakeem Olajuwon in game 5 of a first-round series in the 1985 playoffs. The referees missed it and Olajuwon would only be fined later. However, the Jazz were fired up and came back to win. Paultz pulled down 5 rebounds in the fourth quarter, which incredibly was more than the entire Rockets team got in that quarter. Although the Jazz moved on, they would lose to the Nuggets in 5 games in the next round. Paultz didn't play against the Nuggets, making that game 5 against the Rockets the last of his NBA career.
 
If Raja plays any games for the Jazz this season, he will be the 5th player to play exactly 5 seasons for the jazz. The others:
