Only one 5 year extension from a rookie contract. The CBA has a term called designated player, which is a rookie you can offer the 5 year max extension to.

Technically it`s actually a 6 year extension as those are signed after their 3rd year, which means that the team option for the 4th rookie year is automatically picked up.
