I can't imagine a logo is going to make or break a dental office. I could be wrong though. Seems like most people just find whatever is closest, takes their insurance and has a good reputation.
My opinions: Choice 5 looks a bit like the "beware of radiation" symbol. Choice 4 is a horrendous color. Choices 1, 2, and 3 are all OK, albeit a bit meh. I'd probably go with #2 myself although I can't quite tell what's the symbol inside the g.
Are you guys eco friendly? If not I'd steer away from leafy logos to avoid misleading people. Just my two cents...
Is Green the dentists last name, or a gimmick(companies going green is good for business), or both?
We are very eco friendly.
You don't want to be "too" eco friendly though, u know what I mean? You need stuff to be sterilised and stuff, you don't want to be known for re-using / re-cycling too many things.
I think the Green is your last name angle is a better one. Green Dental - Dr Jacob Green, etc, ...
