Then vs than.

People often use then when they mean than. Then is always related to time. I did this, and then I did that. Than relates to preference. I would rather have this than that. If you say I would rather have this then that, you are saying you want both. You want first one, and then the other.

Bugs me.

"Then" also relates to causality and the notion of superset/subset. A then B can mean that A causes B, of that for A to occur, B must also occur/have occurred.

Point taken, however I think you are mistaking correlation with causality. Because A precedes B does not indicate that A causes B, merely that the occurrence of A is associated with the occurrence of B.

True. I want thinking about the conditional use. So, clearly I should not have used "always". Probably triggered someone else's pet peeve.

What you say is correct. I'm not sure how it is relevant to the different uses of "then", but it is correct.

I don't like if comma then but I try to use it. I also don't like overuse of commas when a sentence flows better without them. Also, if-then is optional and more practical in verbal communication than written. For example "If you finish your homework, only then you can ride your bike.". The comma and then aren't necessary unless you want to emphasize a conditional statement.
